Fill up the form name, date of birth, address, vehicle number, vehicle type, co-passenger name (Only one Co-passenger will be allowed), travel starting point, destination, travel date, time, purpose, return date, time, mobile number, identity card type, identity card number and residing police district. Wait for your application get approved and you will receive pass to your resisted mobile phone
As before, the affidavit can be written or printed out to receive the vaccine and to buy essential items right next to the place of residence. Click here to download
